The purpose of this policy is to provide clear, accessible information regarding The Cortex Collective’s privacy management that will ensure transparency and support your privacy rights.

COLLECTION OF INFORMATION

The information you provide The Cortex Collective may include name, birthdate, address, email address, phone number, credit card details, IP address and billing details (together known as ‘Personal Information’). Note that additional ‘Sensitive Information’ may be collated including an individual’s racial or ethnic origin, political opinions, memberships of a political organisation, religious belief or affiliation, philosophical belief, membership of a professional or trade association, membership of a trade union, sexual orientation or practices, criminal record or health information.

Personal Information is collected for the purposes of:

  • arranging appointments
  • generating invoices
  • delivering effective counselling services with relevant communication

And may be collected via:

  • phone calls or emails or in person administrative discussion
  • booking documentation e.g. client intake and consent forms 
  • website online forms
  • counselling sessions
  • third party service providers when engaged to supply services to a client

ACCESS TO PERSONAL INFORMATION

You may review, correct, update or delete the Personal Information we hold about you, subject to certain legal exceptions. If you wish to access your Personal Information, please contact us in writing. To protect your Personal Information we may require identification from you before releasing the requested information.


HOW WE USE YOUR INFORMATION

By engaging The Cortex Collective via phone, email, and/or our website, or otherwise engaging in counselling services, you consent to the clinician collecting, using and disclosing your Personal Information for the purposes specified in this policy. Collected Personal Information will generally only be used and disclosed for the purpose it was collected. 

Personal Information may be used to:

  • support and improve counselling services
  • administer the website
  • communicate with third party services providers
  • implement our consent form with you
  • identify client logistics through reports and analytics to target client needs 
  • respond to feedback/complaints

DISCLOSURE OF YOUR INFORMATION

We will not disclose, sell, share or trade your Personal Information to any third parties unless you provide consent. Your Personal Information is only disclosed to third party suppliers when it is required by law, for goods or services which you have purchased, for payment processing or to protect our copyright, trademarks and other legal rights. 

If there is a change of control in The Cortex Collective (whether by merger, sale, consolidation, transfer of assets or otherwise) Personal Information could be transferred to one or more third parties under a confidentiality agreement. We would only disclose your Personal Information in good faith and where required by any of the above circumstances.


STORAGE AND SECURITY

Reasonable means will be applied to protect the confidentiality of your collected Personal Information. All Personal Information is stored in a manner that reasonably protects it from misuse, loss, interference, and from unauthorised access, modification or disclosure. Furthermore, reasonable means will be applied to ensure your Personal Information is accurate and complete when collected, used or disclosed by us. Personal information may be stored electronically through third party data centres, which may be located overseas or via third-party secure storage facilities.

Credit card information is not held by The Cortex Collective, rather it is stored with Stripe (please see https://stripe.com/docs/security for more details).

We retain your Personal Information for as long as needed to provide services to you and as otherwise necessary to comply with our legal obligations, resolve disputes and enforce our agreements. When your Personal Information is no longer needed for the purpose for which it was obtained, we will take reasonable steps to destroy or permanently de-identify your Personal Information. 


COOKIES 

A cookie is a small file placed in your web browser that collects information about your web browsing behaviour. Cookies do not access information stored on your computer or any Personal Information (e.g. name, address, email address or telephone number). CHANGE IN PRIVACY POLICY

We may, from time to time, amend this Privacy Policy, in whole or part, in our sole discretion. By continuing to use the Website and/or our service following any changes, you will be deemed to have agreed to such changes. If you do not agree with the terms of this Privacy Policy, as amended from time to time, you must terminate your use of the Website and inform us immediately prior to any further receipt of counselling services.
